Community Health Worker Spotlight

HealthStreet would like to highlight one of its Community Health Workers (CHWs) this month: Carrie Sadler.

Community Health Workers are the core of the HealthStreet model. They work to improve the overall health of the community by providing referrals to medical and social services, health education and transportation. Additionally, CHWs link community members to opportunities to participate in health research. This month, HealthStreet would like to acknowledge one of our passionate CHWs: Carrie Sadler. Through the Florida Blue Grant, Sadler works in Putnam County to connect with community members and provide transportation to medical appointments for those aged 50 and older.

  • Has a Bachelor’s degree in psychology from Saint Leo University.
  • Her goal is to someday obtain a master’s degree in social work and use it to work with battered women and children.
  • Has two daughters and was born and raised in Putnam County.
