About HealthStreet

HealthStreet is an innovative community engagement program with a mission to improve the health of our community by reducing the gaps that exist between health care and health research.

HealthStreet’s community health workers (CHW) assess health concerns, conditions and research perceptions of community members. They also provide referrals to community members for medical and social services, as well as opportunities to participate in health research, relevant to their health concerns and health conditions.
