The Florida Blue Foundation highlighted HealthStreet’s Community Health Worker model in its E-Newsletter for its innovative work in building trust in health research. HealthStreet was the recipient of a three-year $300,000 grant from the Florida Blue Foundation to provide transportation to adults 50 years and older to medical appointments in Union and Putnam Counties.
